For much of the series, Sisko was a commander rather than a captain, though I think he got promoted well before the end. Therefore, beginning-of-TNG-Picard outranks beginning-of-DS9-Sisko. Since Picard keeps refusing getting promoted to admiral, end-of-TNG-Picard does not outrank Sisko's final rank by the end of DS9, even though he easily could've outranked him.

Methinks the circle-thingies are called "pips".

I do not believe so; in the last movie thus far, Star Trek: Nemesis, Riker is planning to leave the Enterprise to become the captain of his own ship.

It's entirely possible that all sorts of crazy stuff happens in the hojillion Star Trek novels; I don't keep track of 'em.

It wouldn't surprise me; there may have been something along the lines of that in the final episode. (I seem to recall an Admiral Riker zooming around in an Enterprise that had an extra warp nacel on its back.) As far as alternate and potential timelines go, many of the senior officers were portrayed as much higher-ranking at some point or another than they actually were in the main timeline of the series before its end. Riker is likely to become an admiral eventually if he doesn't get himself killed first - but it's not inconceivable that Picard himself might not eventually accept the promotions that Starfleet keeps trying to foist upon him as well. There's no knowing how much of any given possible future will come true in the main timeline, since time is always a shifty thing in Star Trek and the future you see is almost never the exact future that you get.
