Most companies have a checking account and a payroll account. Did you get a paycheck that had the itemized deductions on it, or did you just get a check?
Well technically if he paid you under the table and you accepted it, you're both breaking the law. So if you don't get either of the two forms I mentioned within the first week of Feb, I wouldn't mention it to anyone. An employer is required to send you one of those two forms postmarked by Jan 31st otherwise he can face some serious fines. If he paid under the table, then there isn't much of a record that you worked for him/her. By paying you under the table, he isn't responsible to pay for half of certain taxes or for benefits.
